STEPS TO CARE
1.
INITIAL CALL:
Once we receive your information, we will arrange a detailed call to discuss the care situation, address any urgent questions, and schedule a time for an in-person meeting.
2.
IN-HOME CARE ASSESSMENT:
Here, we’ll answer all your questions and ask a few of our own to create a Customized Care Plan tailored to your unique needs.
3.
START OF CARE:
On the first day of care, we’ll accompany our Caregiver to review the Care Plan together and ensure a warm, supportive introduction.
4.
CLIENT VISITS:
After care begins, we’ll occasionally visit during our Caregiver's time with you to check in, see how things are progressing, and assess if any adjustments to the Care Plan are needed.
